MSD Student Travel

*Student Travel/Trip Requests must be submitted to the board for the designated season board
meetings to obtain approval during that season. Post season contests must be included so they are
approved well in advance of the competitions.

Fall 2024: August 21, 2024 Winter 2024: October 30, 2024 Spring 2025: February 19, 2025
Summer 2025: May 28, 2025 Fall 2025: August 13, 2025 Winter 2025: September 3, 2025

The completed Student Travel/Trip Request Form must be received at the District Office by Noon on
the Friday prior to the Regular Board Meeting for inclusion on the meeting agenda for each season.

A Student Travel/Trip Request Form will be considered 'completed' when all fields are populated or
marked NIA if not applicable.

* Additional Teachers, Chaperones are to be listed by name to ensure proper paperwork is on file
at the Building or District Office.

* Trip Planning, Preparations and Precautions must include information on student lodging. In the
case of an athletic team traveling for post-season events, the form must be filled out, in full,

regardless of whether the team has qualified for the competition.

* Authorized Approval Signature must be in place.
